Our History

The Northern Michigan Diabetes Initiative (NMDI) began in 2006 as a collaborative effort between Munson Healthcare, Priority Health, Northern Michigan's Diabetes Outreach Network (TIPDON) and stakeholders from the identified Munson Healthcare geographic service area that includes twelve counties (Antrim, Otsego, Charlevoix, Crawford, Kalkaska, Grand Traverse, Benzie, Leelanau, Manistee, Wexford, Missaukee and Roscommon). The Steering Committee consists of representatives from Munson Medical Center, Priority Health, local physicians, diabetes educators, county health departments and other health and human service representatives.

To get this Initiative started, the Steering Committee made presentations to key regional partners on the Munson Healthcare Community Health Board advocating that diabetes be identified as a critical health care problem in the 11- county region. Providing education and awareness is the primary role of the NMDI. A Community Diabetes survey representative of the 11-county service area provides data that is being used to springboard educational efforts. This survey will be completed again soon so we can see any changes that have been made.

NMDI's efforts change from year to year based on the needs identified at our annual Stakeholder meetings. The input received at this meeting is channeled through the steering committee. The NMDI objectives and small work groups are formed and organized by the NMDI coordinator. Ongoing funding is provided primarily by Munson Healthcare and Priority Health.

The Northern Michigan Diabetes Initiative is a regional collaboration dedicated to prevention, early detection, and management of diabetes.

Our Vision/Goal

A reduction in the prevalence of diabetes and improvement in the care of people with diabetes. We will accomplish this by engaging and providing local, regional, and national resources to develop and implement an improvement plan for the prevention and management of diabetes.

Answer

It is important that people think about diabetes before they get it because diabetes can be prevented and delayed. By making the appropriate lifestyle changes early on people can live healthier lifestyles and prevent or delay diabetes.

Building relationships with community members is an important part of what we do. Our programs help children, adults and seniors so it is necessary for us to connect with those we serve to ensure our programs are useful to the intended audience.
