WASHINGTON (AP) — After months of struggle, House Democrats unveiled sweeping legislation Thursday to extend health care coverage to millions who lack it and create a new option of government-run insurance. A vote is likely next week on the plan patterned closely on President Barack Obama's own.

Speaking on the steps of the Capitol, Speaker Nancy Pelosi said Congress was "on the cusp of delivering on the promise of making affordable, quality health insurance available to every American."

Officials said the measure, once fully phased-in over several years, would extend coverage to 96 percent of Americans. Its principal mechanism is creation of a new government-regulated insurance "exchange" where private companies would sell policies in competition with the government. Federal subsidies would be available to millions of lower-income individuals and families to help them afford the policies, and to small businesses as an incentive to offer coverage to their workers.

Large firms would be required to cover workers, and most individuals would be required to carry insurance.

The ceremony marked a pivotal moment in Democrats' yearlong attempt to answer Obama's call for legislation to remake the nation's health care system by extending insurance, ending industry practices such as denying coverage on the basis of pre-existing medical conditions, and slowing the growth of medical spending nationwide.

Across the Capitol, Senate Democrats, too, are hoping to pass legislation by year's end. Legislation outlined by Majority Leader Harry Reid earlier this week would include an option for a government-run plan, although states could drop out if they wished, a provision not in the House measure.

Obama issued a statement saying House Democrats had reached a "critical milestone" on the road toward a health care overhaul, and singled out the proposed government insurance option. He also said the bill "clearly meets two of the fundamental criteria I have set out: It is fully paid for and will reduce the deficit in the long term."

Republican reaction was swift and critical.

Rep. Tom Price, R-Ga., head of the Republican Study Committee, issued a statement saying Democrats had produced a "government takeover that will limit choice, competition and innovation in health care while increasing costs and decreasing quality." He said the measure would kill jobs, raise taxes and inflict cuts on a program of private Medicare that provides benefits to millions of seniors.

GOP leaders long ago decided to oppose the approach requested by Obama and taken by Democrats, and health care is expected to figure in next year's congressional election campaign.

Democrats issued a statement saying their measure "lowers costs for every patient" and would not add to federal deficits. They put the cost of coverage at under $900 billion over 10 years, a total that didn't include money needed to avoid a scheduled 21 percent cut in doctor fees under Medicare, and omitted other items as well.

With Republicans expected to oppose the measure unanimously, Pelosi and her lieutenants worked for weeks to resolve differences within the Democratic rank and file.

The toughest of them covered the terms under which the government insurance option would function. Liberals generally wanted the government to dictate the rates to be paid to doctors, hospitals and other health care providers, with the fee levels linked to Medicare.

Moderates, fearing the impact on their local hospitals, held out for negotiated rates between the government and private insurers — and won.

Not all liberals were ready to sign on. Rep. Lynn Woolsey, D-Calif., a co-chair of the Congressional Progressive Caucus, was noncommittal about whether progressives would accept the negotiated rates. "This is not walkaway time and it is not acceptance time," she said.

Democrats control 256 seats in the House, are overwhelmingly favored to win one special election next week and are competitive for another. As a result, they can afford 30 defections or more on the legislation and still prevail.

The legislation would be financed by a combination of cuts in planned Medicare spending and an income tax surcharge of 5.4 percent on individuals making at least $500,000 annually and couples making at least $1 million.

The bill would require nearly everyone by 2013 to sign up for health coverage either through their employer, a government program or the new exchange.

In the meantime, a temporary government program would help people turned down by private insurers because of medical problems, lawmakers said. After that, insurers no longer could refuse to provide coverage to the sick, nor could they charge more because of poor health of the insured.

The plan also calls for a significant expansion of Medicaid, the federal-state health program for low-income people. And it would impose a requirement on employers to offer insurance to their workers or face penalties.

Pelosi and the leadership have yet to work out disputes over abortion services and health care for immigrants, issues that must be settled before the bill can come to a vote.

Pelosi has also said the bill would strip the health insurance industry of a long-standing exemption from antitrust laws covering market allocation, price fixing and bid rigging. Democratic officials said the bill also would give the Federal Trade Commission authority to look into the health insurance industry at its own initiative. The officials spoke Wednesday on condition of anonymity, saying they were not authorized to pre-empt a formal announcement.

While precise figures were not immediately available, it appeared the legislation would target the drug industry for more than the $80 billion pharmaceutical firms agreed to contribute toward health care in a deal earlier this year with the White House and key senators. But the industry managed to come away with a provision worth billions: 12 years of market protection for high-tech drugs to combat cancer, Parkinson's and other deadly diseases.

Medical device makers also took a hit, with a 2.5 percent excise tax on sales of their products that is reported to cost the industry $20 billion over the next decade. A $40 billion fee on those businesses was included in a Senate Finance Committee-approved version of the legislation, but Reid is considering cutting it by as much as half.

The republicans lost my vote when they would not put the hammer down on illegal immigration when they had the majority of votes to make it happen and seemingly the will of the public behind them. I realized then that the undercurrents of dirty politics are unsurmountable and hopeless. Our country's best interests have already been sold out. We could have easily protected ourselves from a foreign enemy, but rather we are imploding from within due to our own corruption. I despise GWB for his big spending, the war in Iraq and not being open minded enough to listen to his critics. I am very pleased with his 2 supreme court appointments.

This new plan would basically be Medicaid for everyone. In case you haven't heard, Medicaid turns down 3 times the number of claims as private insurance. All this plan would do is cause a whole lot of Americans to not qualify for medical care while the Hollywood rich (who are all in favor of this plan) laugh at how they have taken over the healthcare system, too.

What if, instead of purchasing the insurance, AIG just covered every american taxpayer when a hospital bill, or an office visit is due?

A bailout can only come back to the taxpayers if the taxpayer is offered a product. Same with GM and Chrysler. The only way to turn a loss into a profit is to GIVE AWAY your service/product.

It's amazing how many selfish people there are out there. They DEMAND that their fellow citizens pay for their health care. They DEMAND that their fellow citizens foot the bill for their retirement.

What happened to personal responsibility? Why should one persons health care or retirement be a burden on their neighbors?

I don't make much money, infact, I qualify for Medicaid and Food stamps. But I refuse to be a burden on my fellow citizens and refuse such services. I have medical bills to pay, which I pay out of MY pocket, not yours. I have a 401(k), which has some money in it, and will be added to in the future, because it is MY responsibility to provide for my retirement, NOT yours.

Then there is the issue of constitutionality...which clearly these redistribution (health care) bills have NONE!

How anybody can think this healthcare bill is not going to destroy our present healthcare, is living in a dream world. I can't even imagine the nightmare it will create.
This government can't even manage a simple "Cash for Clunkers" program that affected a fraction of our population, how in God's name will they manage a complex healthcare plan for 350 million people????
And, even sadder, the $1 trillion price tag is being laid at the feet of our Seniors and our middle class.
I sure hope those of you that support this insanity take a very hard look at how this will change every single visit to your doctor, hospital or clinic, whether it be for a simple cold or flu bug or God forbid, a major issue - like cancer!!! Listen to the majority of folks in Canada or Great Britain!!! They're all saying we're insane if we let this happen to us!!!
There are ways to correct the out of control insurance costs that we all know exist, but destroying our healthcare system as we know it, is not the answer. I pray with all my heart that our Federal politicians will not let Pelosi, Reid and Obama win this one. This will not be a win for America, this will be a win for the very Liberal Left only.
Nothing is free!!! And if you think this is a free lunch to medical care, you are sadly mistaken. We will destroy the best healthcare system in the world and that will be a sad day for the world!!!

My freedoms are being taken away, by not having the right to do what I want with my body. If I don't want health insurance, no one should have the right to force me to pay for it. I have had jobs before that have offered it, but if you opt out, you got more money. It was my choice. That's what it should be about is choice. To be taxed extra at the end of the year because you don't have health insurance is wrong. And if people are ok with this now, for the good of insurance, what will happen when the government says, you have to pay more if you are overweight? Or what if you are a carpet layer and you have bad knees from it? Then you have to pay more. If the people let the government dictate what you have to have and/or do with your own body, there is no telling where it will end. As far as I am concerned no one has the right to tell me I have to have health insurance, it is taking away my freedoms, and it is the start of bad things to come.

It doesn't matter who gets sent to Washington, once they are in the "club" they are all the same. For years they have become insulated from the peasants who elect them and have created nice little perks and retirement packages for themselves. In order to keep their job security/name recognition in the public eye, they have squandered our money on little "thank-you's" to the folks back home....bridges to nowhere, parks,museums,airports,libraries, programs of all kinds,...all very "nice" ways of saying "I love you" with the taxpayer's money. The recipients of these tokens include: sweet little grandmas and grandpas, dedicated educators, contractors and healthcare facilities,insurance companies, administrators of all fields, etc, who legally milk the government for all they can get. Individually they do not resist the "assistance" because they know that the printing presses are humming and if they don't take the money, someone else will. The motto is " I don't know what it is... but if it's free, I want it!" The cumulative effect of all these handouts is financially devastating. Here we are in 2009, mired down in 2 senseless wars and a healthcare crisis that is bankrupting the middle class, it's a shame that our government decided to waste our money on bubble gum and cotton candy.

She sits on her throne and states "they have listened to the people,"(with just a hint of arrogance) effectively shoving this 2000 page bill of nonsense up every American's arse deep where the sun don't shine. Let's fire the poloticians and make our own decisions. We certainly can't do any worse. You all talk about health reform and our government trying to make healthcare affordable. I don't know about other states but here in michigan I have heard and know of people on medicaid who are required to pay astronomical co-pays or deductables before medicaid kicks in. Hell one person I know of has to pay out over $800.00/month before medicaid pays. You can get a health polocy for less then that.
This is our rep's watching out for our interest.I'm a disabled veteren and as far as freedoms, I'm not sure what I was fighting for anymore.

This power grab is nothing like car insurance. You have car insurance incase you damage someone or their property. And saying since this is America we should all be just tickled to death to pay more taxes. Is totally against the morels this country was founded on. We should help each other and find ways to care for anyone that needs it. But haveing the government do it will not work. Medicare is a bust and they want to do that for everyone. Go to debt clock .org and see how Medicare is doing.

If tax came from everyone who uses the government insurance it might work the problem is TONS, MILLIONS of people will get the insurance without paying in, heck only about half the people pay federal income tax anyways. Quit wanting something for free. I'm sick of people wanting other people to pay for their things, which is a sick part of human nature. If your kid was in need of medical care would you rob your neighbor to pay for it? I wouldn't because stealing is wrong. I don’t want a GD thing for free. There are things the government could do to lower insurance costs like tort reform, exchanges, fewer limits on competition, but none of that crap is in this bill. You people are hoping to use the government to rob your neighbor, SICK.

Just like Car insurance! Everybody has to pay to get it. everybody has to pay for medicare social security and so on. How would you like not to have social security when your old and stinky. No body complains about social security,car insurance and so on because everything has a purpose. We need healthreform more than anything. Long overdue. Health is number 1. Not everybody is rich and can afford health insurance. This is the United States of America incase you havent heard. everybody should have to pay more taxes along with social security ect. for every American to have Health Insurance. It would be more expensive not to have Health reform. Instead of complaining about higher taxes for health reform, think of it as a health insurance saving account for when you do need medical attention you will be covered just like YOUR NEW CAR. Oh wait , your car is more important than your health??????I think health insurance for every one is important than anything else. We all should pay taxes on this much needed Health insurance for all
